To finalize your equipment rental, we require a completed Insurance Verification and Liability Waiver. These standard documents outline safety responsibilities, protect both parties during the rental period, and ensure proper insurance coverage is in place before the equipment is delivered or picked up.

Please complete, sign, and return these documents at least 24 hours before your scheduled rental.

IF USING A TRAILER YOU MUST HAVE A MINIMUM 1 TON PICKUP TRUCK USED FOR TRANSPORTING EQUIPMENT. NO EXCEPTIONS
